Job Title: Forensic Disability Transition Specialist

About the Role

This role works collaboratively with a multidisciplinary team to provide clinical assessment and treatment. The successful candidate will work in various settings, including prisons and office locations.

Key Objectives:
  • To identify suitable clients for intensive rehabilitation programs as early as possible.
  • To support transition of clients to maximize their success in forensic services.
  • To work with complex clients directly or with their care teams as they transition out of custody.
  • To upskill staff and stakeholders to work with clients transitioning out of custody.
Accountabilities:
  • To provide advice to key prison staff on identifying and referring suitable clients to intensive rehabilitation programs.
  • To maintain collaborative relationships amongst Corrections Victoria, FDS, and external stakeholders.
  • To develop sound transition processes for clients with intellectual disabilities being released from prison.
