Role Overview
This position is part of St Vincent's Hospital Melbourne's (SVHM) Health Independence Program, which aims to deliver improved outcomes for clients and support hospital demand management and flow. As a Social Worker Grade 3 in our Complex Disability Service, you will work in a multidisciplinary team to provide high-quality care and support to clients with complex needs.
Key Responsibilities
- Delivery of client-focused, high-quality referral triage, assessment, care planning, referral management, reviews, counselling, and carer support within a caseload.
- Collaborative work with the multidisciplinary team, referring professionals, community professionals, and other key stakeholders within a flexible structure.
- Effective communication of client issues with team members and contribution to multidisciplinary team meetings.
Requirements
- Bachelor of Social Work or relevant qualification.
- Minimum of seven years' post-graduate experience in social work, preferably in a disability or healthcare setting.
- Eligibility for membership of a relevant discipline association.
- Post-graduate qualifications and experience in a similar field are highly desirable.
- Current National criminal history check and Working with Children check.
