Social Worker

We are seeking an experienced Grade 2 Social Worker to join our team at St Vincent's Hospital Melbourne. As a key member of our acute medicine team, you will work closely with patients, families, and carers to provide comprehensive psychosocial assessments, develop intervention plans, and facilitate discharge processes.
Key Responsibilities- Conduct multi-dimensional assessments and develop complex intervention plans
- Liaise with patients, families, and carers to coordinate treatment and outcomes
- Participate in quality improvement and research activities
- Provide supervision and participate in clinical education programs
- Promote positive relationships within the department and with community partners
- Bachelor Degree or Qualifying Masters in Social Work
- Significant social work experience in a hospital or related inpatient healthcare setting
- Current National criminal history check, or willingness to obtain
- Current Working with Children Check desirable
