About the Role
We are seeking an experienced Grade 2 Social Worker to join our team at St Vincent's Hospital Melbourne. As a key member of our acute medicine team, you will work closely with patients, their families, and carers to provide comprehensive psychosocial assessments and develop complex intervention and discharge plans.
Key Responsibilities
- Undertake multi-dimensional, comprehensive psychosocial assessments and formulate appropriate complex intervention and discharge plans
- Liaise with patients, their families, and/or carers to co-ordinate, facilitate, and communicate patient treatment and outcomes
- Actively participate in quality improvement and research activities
- Provide supervision and participate in clinical education programs as required
- Promote and develop positive relationships within the department, wider organisation, and with community partners
Requirements
- Bachelor Degree or Qualifying Masters in Social Work
- Significant social work experience in a hospital or related inpatient healthcare setting is essential
- Current National criminal history check, or willing to obtain
- Current Working with Children Check desirable
