Job Overview:We are seeking an experienced Palliative Care Specialist to join our dedicated team. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and support.
Key Responsibilities:- Provide holistic care to palliative care clients, including outpatient and inpatient support, with a focus on ensuring individualized care plans meet each patient's unique needs.
- Collaborate with occupational therapists, social workers, dieticians, oncology specialists, and community nursing teams to ensure seamless care coordination and outcomes.
- Support patients and families in making informed decisions, including advanced care planning, to ensure their preferences and values are respected.
- Ensure care is culturally sensitive and tailored to each individual's needs, promoting dignity and respect for all patients.
