Job Summary
">A compassionate and skilled healthcare professional is required to join our dedicated palliative care team in Moree, NSW. As a valued member of our team, you will deliver holistic care to patients across the region, including Mungindi, Boggabilla, Toomelah, and Warialda.
">- About the Role:
- Deliver high-quality, patient-centered care to individuals and families within the community.
- Work closely with Occupational Therapists, Social Workers, Dieticians, Oncology specialists, and Community Nursing teams.
- Support patients and families in making informed decisions, including advanced care planning.
- Ensure care is compassionate, culturally sensitive, and tailored to each individual's needs.
- Provide outpatient and inpatient support to palliative care clients.
- Collaborate with multidisciplinary teams to develop and implement comprehensive care plans.
- Conduct regular assessments and evaluations to ensure optimal patient outcomes.
- Communicate effectively with patients, families, and healthcare professionals to promote seamless care coordination.

Requirements
">- Degree in Nursing or relevant healthcare discipline.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Proven experience working in palliative care or a related field.
