Job Title: Caring Healthcare Professional

This role requires a caring and compassionate individual to provide holistic care to patients in Palliative Care. The successful candidate will be part of a dedicated team delivering high-quality patient care.

Key Responsibilities:
  • Deliver patient-centered care to individuals with complex needs, providing emotional support and guidance throughout their treatment journey.
  • Collaborate closely with Occupational Therapists, Social Workers, Dieticians, Oncology specialists, and Community Nursing teams to ensure comprehensive patient care.
  • Support patients and families in making informed decisions, including advanced care planning, ensuring they feel empowered and confident throughout the process.
