Key Leadership Position

This is a pivotal role in planning, managing and leading care services across various programs.

Essential Responsibilities:
  • Coordinate and manage care services to ensure the highest standards of client care.
  • Develop tailored care plans for individual clients to meet their unique needs and goals.
  • Collaborate with stakeholders, including clients, families, healthcare professionals, to provide comprehensive care support.
  • Establish strong relationships with clients, team members and stakeholders to foster trust and effective communication.
  • Ensure compliance with relevant legislation and organizational policies to maintain quality and efficiency.
Required Expertise
  • Demonstrate experience in case management and client care coordination in a Community Aged Care setting.
  • Display strong verbal, written and interpersonal communication skills to work effectively with diverse people.
  • Provide proof of immunity or willingness to become vaccinated against certain diseases.
  • Hold a current Police Screening Certificate and First Aid Certificate.
  • Current NDIS Worker Screening Check (Yellow Card) and Blue Card (Working with Children Check).
  • Current driver's licence and tertiary qualifications in a health-related field.
