About this Opportunity:

As a Care Manager, you will be responsible for overseeing and coordinating all activities related to client care. This role entails leadership and management of the overall client experience, ensuring effective coordination with the office team.

The Care Manager will have the client's best interests at heart while supporting operational goals. Key responsibilities include planning, managing, coordinating, and reviewing client objectives, communicating with team members and clients to develop care plans, and documenting interactions with clients, family, or other stakeholders.
