We are seeking an experienced Family Support Specialist to join our Targeted Earlier Intervention team.

The successful candidate will provide case management support to families with children under 18 years old living in the area.

You will work collaboratively with families to develop and implement individualised case plans, focusing on enhancing family functioning, support with improving family relationships and connections, and building capacity of families to address their own needs.

Key Responsibilities:
  • Providing tailored case management support to families
  • Developing and implementing individualised case plans
  • Working collaboratively with families to achieve their goals

Requirements:

  • Minimum requirement of a tertiary qualification in community services or other relevant areas and at least 3 years relevant case management experience
  • Demonstrated experience in providing holistic case management support focusing on strengths and needs
  • Strong communication, interpersonal, organisational & problem-solving skills

About the Role:
