Care Coordinator Leader

We are seeking a dynamic, highly motivated and enthusiastic professional to lead our service in Geelong.
This is an opportunity to spearhead a service with significant growth and a thriving new home maternity unit, working collaboratively with healthcare providers to deliver high-quality patient care.
Key Responsibilities- Leverages expertise to foster a culture of continuous improvement where all patient interactions drive memorable patient experiences, underpinned by excellence and safety in care.
- Monitors, analyses, and reports variances and trends in clinical data to continuously improve clinical quality and safety outcomes.
- Promotes policy and procedure development to optimise clinical outcomes and enhance the patient experience.
- Acts to integrate service models with healthcare providers to deliver seamless care in the community and ambulatory setting.
- Promotes evidence-based practice, including research and quality activities to improve patients' care in the home setting.
- Participates in and implements quality improvement and risk management into all aspects of service.
- Registered Healthcare Professional with current registration.
- Tertiary qualification in healthcare, management and/or relevant discipline.
- Recent acute clinical experience in home healthcare.
- Valid driver's license, Working with Children Check and National Police Clearance.
