Dedicated Home Care Team Leader

Key Responsibilities- Coordinate care in an allocated section of home, ensuring care plan compliance throughout the shift.
- Advise and allocate care staff, recognizing and escalating deteriorating residents as needed.
- Provide safe, high-quality care in accordance with Aged Care Quality Standards, promoting a positive resident experience through a resident-first approach.
- Maintain documentation as per organizational standards.
- Registration with AHPRA as an Enrolled Nurse is mandatory.
- One year of experience as an Enrolled Nurse in a residential aged care setting is desirable (new entrants will be considered).
- Strong verbal and written communication skills.
- Hold NDIS Worker Checks (or willing to obtain).
