Case Manager Job Description

We are seeking a dedicated professional to fill the role of Case Manager at one of our residential facilities.

  • The ideal candidate will have strong interpersonal skills and the ability to build effective relationships with clients, internal teams, and external agencies.
  • They will provide personalized support and advocacy for clients, ensuring they access necessary health, housing, legal, education, and well-being services.
  • This involves creating and implementing case plans that address clients' unique needs and goals.
  • The Case Manager will also be responsible for maintaining accurate documentation, contributing to service quality, and collaborating with colleagues to ensure coordinated care.
