Job Description

The HCP Care Partner role is a critical position within our organization, focusing on delivering exceptional person-centered support to our clients. This involves building meaningful relationships with internal and external stakeholders to ensure that clients receive the care and support they need to live their lives as they choose.

This role requires coordinating services within established guidelines, implementing client service responsibilities in line with client needs, and ensuring compliance with relevant standards and regulations. Additionally, conducting assessments and reviews as required, and undertaking intake of new clients are essential aspects of this position.

Required Skills and Qualifications

To succeed in this role, you will need:

  • Experience in a similar role and relevant qualifications
  • Knowledge, understanding and experience in Aged care, disability and/or Home and Community care
  • A genuine desire to deliver outstanding customer service in all interactions
  • Strong communication and interpersonal skills with a focus on building and fostering meaningful relationships
