Job Summary

This role is centered around delivering exceptional person-centered support to clients. The HCP Care Partner works collaboratively with internal and external stakeholders to ensure clients receive the care and support they need to live their desired lifestyle.


The Role Responsibilities
  • Coordinate services within established business guidelines.
  • Implement client service responsibilities in accordance with client needs.
  • Ensure Aged Care Quality Standards and HCP compliance requirements are met.
  • Conduct assessments and reviews as required, and undertake intake of new clients.

You will be a supportive, friendly, and organized individual with excellent time management, leadership, and customer service skills. You will thrive in a dynamic and innovative environment and enjoy being part of a highly skilled team who believe in the care and service they provide.


  • Experience in a similar role and relevant qualifications.
  • Knowledge, understanding, and experience in Aged care, disability, and/or Home and Community care.
  • A genuine desire to deliver outstanding customer service in all interactions.
  • Strong communication and interpersonal skills with a focus on building and fostering meaningful relationships.
