Our broad range of primary health services, care management and in home and community support services can be tailored to an individual's needs and with major hubs in Box Hill and Eltham, these services can be delivered locally, in the Eastern and North Eastern suburbs of Melbourne.

Job DescriptionThe opportunity:
We are looking for a Health Navigator to join our supportive and professional multidisciplinary teams on a fixed term contract (0.6FTE).
Responsibilities include but are not limited to;
- Undertake service provision in accordance with the National Clinical Assessment Framework for Children and Young People in out of home care.
- Provide triage and support to young people who enter Child Protection, including: triaging health needs, providing 1:1 and group supports, support staff through secondary consultation, and other supports as relevant.
- Refer appropriate children and young people to the PTGH clinic for multidisciplinary assessment and support
- Provide secondary consultation with the staff supporting the children/youth in the out of home care setting house as necessary.
- Undertake health and wellbeing assessments with children and adolescents in Out of Home Care
- Collaborate with child protection services, paediatricians and allied health professionals to achieve the best health outcomes for children in Out of Home Care.
- Work collaboratively and in consultation with stakeholders such as support staff, Child Protection and internal and external supports to ensure children and young people participating in the program have health and safety requirements met; and are linked into appropriate services to improve health, safety and social outcomes.
- Work collaboratively with the client and their carer/s to ensure that they are actively engaged and involved in making decisions about their life, goals, and the care they receive.
Our ideal candidate:
- Qualified Division 1 Nurse and / or a relevant tertiary qualification in Allied Health, including Social Work, Speech Therapy, Occupational Therapy, Psychology, or related discipline (with appropriate health-related experience).
- Registration with Australian Health Practitioner Regulation Agency (AHPRA) or equivalent
- Relevant experience working with children and young people aged 0-17
- Full Driver's License
- Experience in triage and coaching children and young people to achieve improved health outcomes
- Experience working with children and young people with complex experiences of vulnerability, disadvantage and trauma.
- Experience embedding trauma-informed care in practice with young people, their care givers and other supports.
- Experience working holistically with children, young people and their supports
- Strong understanding of local health systems and referral pathways to connect children and young people with best supports to improve their health and wellbeing
- Experience working with multidisciplinary allied health teams.
- Demonstrated capacity to establish, manage and maintain internal and external stakeholder relationships.
