Nurse Care Coordinator

Job Summary:
We are seeking a skilled Registered Nurse to join our team. As a Registered Nurse, you will be responsible for coordinating and delivering high-quality person-centered nursing care in accordance with the Nursing and Midwifery Board of Australia Standards for Practice for Registered Nurses, and the Policies and Procedures of the Ministry of Health (MoH) and Mid North Coast Local Health District (MNCLHD).
Key Responsibilities:
* Coordinate and deliver high-quality person-centered nursing care in accordance with the Nursing and Midwifery Board of Australia Standards for Practice for Registered Nurses, and the Policies and Procedures of the Ministry of Health (MoH) and Mid North Coast Local Health District (MNCLHD).
* Work in a multidisciplinary team approach in the planning, delivery, and coordination of patient care.
* Demonstrate effective communication through the use of technology, written, verbal, and interpersonal skills with the ability to identify key messages, issues, and concerns when communicating with others.
* Have knowledge and understanding of person-centered care, evidence-based practice, and quality improvement.
* Be available and capacity to participate in a 24-hour/seven-day rotating roster which includes working all shifts, all days of the week including public holidays.
Requirements:
* Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1).
* Demonstrated recent clinical experience in emergency, critical care, or relevant acute hospital settings.
* Demonstrated ability to use problem-solving skills and a multidisciplinary team approach in the planning, delivery, and coordination of patient care.
* Demonstrated effective communication through the use of technology, written, verbal, and interpersonal skills with the ability to identify key messages, issues, and concerns when communicating with others.
* Knowledge and understanding of person-centered care, evidence-based practice, and quality improvement.
* Demonstrated understanding of and ability to practice in accordance with legislation, policies, and within the scope of practice of a Registered Nurse.
