
Youth Homelessness Support Specialist

**Job Opportunity:**
Support Worker - Outreach ServiceWe are seeking a qualified and compassionate support worker to join our outreach team in providing exceptional case management services to young people aged 15-25 experiencing homelessness or at risk of homelessness.
This is a unique opportunity to work with a diverse group of young people, engaging them in weekly assertive outreach within the city and advocating on their behalf at relevant networks and communities.
Key Responsibilities:- Employ a person-centred approach in working with young people who are experiencing complex and co-occurring needs.
- Build strong, professional relationships with young people, providing ongoing support and guidance.
- Develop and implement individualised case management plans, focusing on safety planning, assessment, and crisis intervention skills.
- Deliver an outreach service to street-present young people, connecting them with essential resources and services.
- Collaborate with internal and external stakeholders to ensure seamless support for young people.
- Current National Police Clearance valid within one year.
- WA Working with Children Check.
- Senior First Aid certificate.
- WA Driver's Licence with a willingness to obtain an MR bus licence.
About the Role:
