Key Responsibilities- Provide personal care and support to individuals with disabilities, including assistance with daily living activities, mobility, and communication.
- Develop and implement individualized support plans to meet the unique needs and goals of each client.
- Work collaboratively with a care team approach, including guidance from professionals, to ensure comprehensive support and care.
- Assist with domestic duties, such as cleaning, laundry, and meal preparation, to promote independence and well-being.
- Provide transportation and support for clients to access community activities, appointments, and social events.
- Monitor and report any changes in client behavior, health, or well-being to ensure timely interventions and support.
- Experience working with individuals with disabilities, particularly those with complex needs.
