Our organization seeks a highly skilled Clinical Case Coordinator to join its team.
Key ResponsibilitiesThe successful candidate will play a pivotal role in supporting the Child and Youth Assessment & Therapeutic Service (CYATS) by ensuring that children/young people and their families receive responsive services.
Liaising with key stakeholders, the Clinical Case Coordinator will collect background information about clients, manage administrative tasks, and provide clinical case management support to vulnerable families.
To excel in this position, you will require:
- A minimum of diploma qualified or relevant experience in a clinical case coordination role or similar
- Demonstrated expertise and skills in providing clinical case management support to vulnerable families that is child-focused, family-centred, and trauma-informed
- Knowledge of child development, as well as neurodevelopmental disorders including FASD, ASD, and Global Development Disorders
This is an excellent opportunity to work collaboratively with a multidisciplinary team and contribute to the provision of seamless assessment, treatment, management, and support access to the National Disability Insurance Scheme.
