Child Health Outreach Program Nurse/AHP

We are seeking a skilled and dedicated professional to join our team as a Child Health Outreach Program Nurse/AHP.
About the RoleThe successful candidate will be responsible for working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes.
The position has a strong clinical focus on outreach work including liaison with internal and external stakeholders involved in the child and family's care.
Key Responsibilities- Coordinate care to optimise health outcomes for children with complex chronic healthcare needs;
- Work collaboratively with children 0 – 15 years and their families to facilitate and support their healthcare needs;
- Liaise with internal and external stakeholders involved in the child and family's care;
- Develop and implement effective care plans to meet the needs of children and their families;
- Provide education and support to children and their families to promote healthy lifestyles and disease prevention.
- Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA);
- Demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health;
- Working knowledge of Child Health and an understanding of current practices, theories and values;
- Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal People;
