Base Salary: $103,759 - $110,838 (p.a.)

Role Overview:
The Child Health Outreach Program Nurse/AHP plays a crucial role in the Child Health Outreach Program, collaborating with children aged 0 - 15 years and their families. This position is dedicated to facilitating and supporting children with complex chronic healthcare needs through effective care coordination aimed at optimizing health outcomes. The role emphasizes outreach activities and requires close collaboration with both internal and external stakeholders involved in the care of the child and family.
Candidate Profile:
- Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
- Proven understanding of primary health care and developing clinical expertise in chronic disease management and/or child health
- Familiarity with Child Health practices and a grasp of contemporary theories and values
- Commitment to Aboriginal health and the principles of Aboriginal Community Control, along with awareness of current issues impacting Aboriginal communities
