Child Health Outreach Program Nurse/AHP Specialist

The Child Health Outreach Program Nurse/AHP is a critical position within the Child Health Outreach Program, working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes.
This role has a strong clinical focus on outreach work, including liaison with internal and external stakeholders involved in the child and family's care.
About YouTo be successful in this role, you will require:
- Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA);
- Demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health;
- Working knowledge of Child Health and an understanding of current practices, theories and values.
- Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal People;
