We are seeking a dedicated individual to join our team as a Care Coordinator at Right at Home Southern NSW. This role is a wonderful opportunity to make a difference in the lives of our valued clients and their families. As a Care Coordinator, you will be responsible for building strong connections with clients and their families, assessing client needs and resources, and conducting Client Care Assessments and Care Planning.

Key Responsibilities:

  • Client Care and Support: You will be responsible for building strong connections with clients and their families, assessing client needs and resources, and conducting Client Care Assessments and Care Planning.
  • Care Planning and Coordination: You will work closely with our care team to develop and implement individualized care plans that meet the unique needs of our clients.
  • Communication and Collaboration: You will be the primary point of contact for clients and their families, providing them with information, support, and guidance throughout the care planning process.

Requirements:

  • Experience: You will need to have at least 3 years of experience in a care coordination or related role.
  • Qualifications: A Certificate I - IV in a related field is preferred, but not required.
  • Skills: You will need to have strong organizational, administrative, and time management skills, as well as the ability to work independently and as part of a team.
  • Personal Qualities: You will need to be compassionate, empathetic, and able to communicate effectively with clients and their families.
