Job Opportunity: Support Coordinator for NDIS Participants

Sunflower Community is seeking a dedicated and passionate Support Coordinator with experience in working with NDIS participants.

We deliver a range of services aiming to create a positive impact on people's lives, ensuring participants achieve their goals and live independently.

  • Interpret and implement NDIS plans within the framework, mainstream, and community services, maximizing participant outcomes.
  • Manage multi-disciplinary teams and services for participant support coordination, providing holistic care and solutions.
  • Identify and engage suitable supports, work towards achieving outcomes and goals, empowering participants to self-manage their lives.
  • Holistically address crisis points, barriers, and participant risks, designing solutions to manage these risks and promote stability.
  • Build strong relationships with government and non-government bodies, service providers, allied health professionals, and practitioners, fostering collaborative approaches.
  • Coordinate stakeholders for collaborative approaches across all areas of support, prioritizing participant needs and goals.
  • Help participants develop self-direction, self-advocacy, working with professionals, and managing their lives and NDIS plans according to their choice.
  • Maintain thorough case-note recording and prepare reports as needed in line with NDIS Support Coordination requirements, ensuring transparency and accountability.
About You
  • A minimum 12 months of experience in Support Coordination or Local Area Coordination (LAC), identifying and linking participants to diverse support services.
  • Excellent interpersonal skills that value individual and cultural diversity, fostering productive relationships with various stakeholders.
  • A strong solution focus, creativity, and ability to think outside the box, addressing complex challenges and finding innovative solutions.
  • Demonstrated ability to provide a range of solutions to complex challenges, barriers, and risks facing participants, working with stakeholders to manage such challenges.
  • Experience working collaboratively with a small team, showing excellent communication skills and willingness for continuous learning.
  • In-depth knowledge of the National Disability Insurance Scheme (NDIS) rules and operational guidelines, experience coordinating community-based supports to achieve participant goals.
