
Community Care Specialist

This is an exciting opportunity to join our dedicated Rehab@Home team and provide compassionate, person-centred nursing care in community and residential settings.
About Your Role:
- Conduct comprehensive nursing assessments and develop tailored care plans that meet the unique needs of each patient.
- Administer clinical care such as wound management, catheter care, and medication administration with precision and care.
- Educate patients and their families on self-care and health management techniques, promoting independence and confidence.
- Work independently and collaboratively with nursing staff and allied health professionals to deliver high-quality patient outcomes.
- Support quality improvement initiatives and maintain accurate documentation in electronic medical records.
- Participate in on-call roster and occasional work across multiple sites within Metro South Health.
- Lead and supervise junior nursing and care staff as required, providing guidance and mentorship to help them grow in their careers.
About You:
- You're a dedicated and compassionate Registered Nurse who thrives in a community care environment, passionate about delivering person-centred care and working collaboratively with others.
- You take pride in your clinical competence and ability to provide care in a community or similar setting, with excellent assessment, planning, critical thinking, and communication skills.
- To succeed in this role, you will bring:
- Demonstrated experience in community-based rehabilitation and restorative care, with a strong commitment to evidence-based practice.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- A current Class C driver's license and willingness to travel across Metro South Health sites, with flexibility to adapt to changing priorities and schedules.
