We are seeking a skilled and compassionate Aboriginal Torres Strait Islander social worker to join our team in delivering high-quality patient care.
Job DescriptionThis is an exciting opportunity for a dedicated professional to provide psychosocial assessments and complex discharge planning for oncology inpatients. Working closely with medical teams, you will deliver patient-centered care in a supportive part-time role.
- Conduct thorough psychosocial assessments for oncology inpatients.
- Lead multidisciplinary discharge planning efforts.
- Offer support to patients, families, and carers.
To succeed in this role, you will need:
- A degree in Social Work.
- Current or eligible AASW registration.
- Strong skills in discharge planning and assessments.
- Fully vaccinated for COVID-19 (including booster).
