About the Role

We're seeking an experienced Support Coordinator to join our team. As a Support Coordinator, you'll work within the Department of Health and Human Services (DHHS) programs and the National Disability Insurance Scheme (NDIS) framework to provide leadership in coordination and transition supports for people with disabilities.

You'll contribute to the development of service practice through regular staff meetings, planning processes, and policy reviews. Your role will also involve creating programs that support the transition to person-directed services, including exploring alternative support options with participants and their support networks.

Requirements
  • Demonstrated commitment to Annecto's purpose and principles
  • Knowledge of the NDIS and the disability sector
  • Understanding and commitment to person-centered approaches
  • Able to work efficiently with culturally diverse groups
  • Able to work autonomously with a high degree of initiative
  • Excellent communication skills
  • Previous experience in support coordination in a community setting
  • Current Victorian driver's license and NDIS worker screening check
Preferred Requirements
  • Experience working in a similar capacity in a community-based not-for-profit organization
  • Knowledge of client databases, such as Alayacare
  • Training and experience in person-centered planning
  • Able to speak a community language

We offer a generous salary packaging option, with a salary range of $73,799.72 - $79,131.80 per year, plus 11.5% superannuation.
