Home Care Program Leader

We are seeking an experienced and passionate professional to lead our dynamic team in delivering high-quality care services.

  • About the Role:
  • A Key Position Within Our Organisation
  • This leadership position is responsible for overseeing the home care package program, working closely with senior management to ensure effective delivery of services.
  • The successful candidate will be responsible for monitoring client outcomes and service effectiveness, identifying areas for improvement and implementing changes as needed.
  • To achieve these goals, the Team Leader must be able to build strong relationships with clients, family members, healthcare providers, and community organisations.
  • Additionally, they will be responsible for ensuring compliance with data collection requirements and funding targets.
  • Main Responsibilities:
  • Lead and coordinate the Home Care Package Program
  • Monitor client outcomes and service effectiveness
  • Build and maintain relationships with key stakeholders
  • Ensure compliance with data collection requirements and funding targets
  • Identify and implement service improvements to enhance care quality and competitiveness
  • Actively manage compliments, complaints, and feedback, striving for continuous improvement

Skill Requirements:

  • Minimum of 3 years' experience in aged care, home care, or a related discipline
  • Sound knowledge of the Home Care and/or Aged Care industry, including the Aged Care Quality Code of Conduct, Aged Care Standards, and accreditation processes
  • Strong communication skills with the ability to build rapport with internal and external stakeholders
  • Experience in budgeting and fund management related to Home Care Packages
  • Willingness to travel both intra and interstate
  • Availability to participate in after-hours on-call duties
