Job Description

We are seeking a skilled professional to manage the transition of new clients into our organization. The successful candidate will be responsible for ensuring that all clients receive an exceptional experience, fostering trust, transparency, and effective communication.

The ideal candidate will have strong project management skills, drive, and passion for patient-centered care. They will oversee client transitions safely, effectively, and within timeframes. They will also build strong relationships with clients and stakeholders, and maintain oversight over transition project trackers.

This role requires strong analytical skills, attention to detail, and excellent communication skills. Ideal candidates possess a Bachelor or postgraduate degree in an Allied Health Profession, significant disability and healthcare operational experience.
