We are seeking a highly skilled Clinical Nurse Specialist 2 to join our team in the Acute to Aged Related Services (AARCS) team.
Job SummaryThe AARCS team provides specialized care for older patients in acute inpatient settings who have complex care needs. The successful candidate will be responsible for delivering advanced clinical care, leadership and expertise in managing complex cases.
This role involves providing patient-centered, comprehensive assessment, care planning and coordination to enable safe and timely discharges. Key responsibilities include:
- Providing advanced clinical care for older patients and leading complex case management.
- Delivering the first face-to-face assessment within one working day of referral.
- Collaborating with families, carers and service providers to develop safe discharge plans.
- Ensuring that the care provided is coordinated and effective, resulting in improved patient outcomes.

The ideal candidate will have current registration as a Registered Nurse with AHPRA and post-registration qualifications and experience appropriate to the CNS2 role.
