  • Delivering high-quality acute care interventions and supporting clients recently discharged from inpatient services.
  • Responding promptly to those presenting with urgent care needs.
  • Collaborating closely with clients, their families, and carers to develop evidence-based treatment plans.
  • Effective teamwork and coordination with inpatient units, community-based services, and other members of the Access team to ensure continuity and quality of care.
