Complex Needs Case Manager

At our organization, we strive to provide high-quality support services to the community. As a Homelessness Support Specialist, you will be part of our mission to end homelessness by providing support for individuals with complex needs.
The role involves engaging with vulnerable individuals and communities, working with Aboriginal groups, and providing culturally sensitive support. You will also participate in outreach efforts to resolve homelessness issues.
To succeed in this role, you will need:
- Demonstrated experience in working with homeless or at-risk populations
- Experience in working with Aboriginal communities and the ability to provide culturally appropriate support
- Demonstrated experience in working with people impacted by substance abuse and/or mental health conditions
- A good understanding of local health systems and referral pathways
- A minimum 2 years' experience in case management, preferably in healthcare or social services
You will also require:
- Tertiary qualifications in Social Work, Human Services, or a related field, or significant relevant experience
- Current or willingness to obtain a Working with Children's Check (WWCC)
