Emergency Department Youth Navigator Position Overview

We are seeking a skilled professional to work in our emergency department, providing specialized support to adolescents and young adults.

About the Role:

The successful candidate will conduct assessments, develop care plans, and collaborate with multidisciplinary teams to provide holistic support. They will also be responsible for health education and connecting clients with relevant community resources and follow-up services.

Key Responsibilities:
  • Conduct comprehensive assessments of adolescents and young adults accessing emergency department services.
  • Develop tailored care plans to meet the unique needs of each client.
  • Collaborate effectively with healthcare professionals from various disciplines to deliver comprehensive care.
  • Provide evidence-based health education and promote healthy behaviors.
  • Connect clients with relevant community resources and follow-up services to ensure continuity of care.
Requirements:
  • Clinical expertise in assessment, care planning, and case management.
  • Excellent communication, interpersonal, and advocacy skills.
  • Ability to work collaboratively within multidisciplinary teams.
  • Demonstrated knowledge of youth health issues and mental health first aid principles.
  • Experience in health education and promotion, as well as documentation and record-keeping.
  • A certified healthcare professional (such as Registered Nurse or Social Worker) is highly desirable.
