Western Sydney Local Health District is seeking a Youth Health ED Navigator for a full-time, on-site role based in Sydney. The successful applicant will provide direct support to adolescents and young adults accessing emergency department services.
We are looking for an experienced healthcare professional who can conduct assessments, develop care plans, collaborate with multidisciplinary teams, provide health education, and connect clients with relevant community resources and follow-up services.
