About Us

We provide services for NDIS participants and support them to reach their goals.

Our mission is to uphold independence and ensure that all people with a disability have their needs met.

The role involves finding referrals for supports and services required by participants, their families on an individual basis.

  • Coordinate referrals for NDIS participants including mainstream, informal and formal support.
  • Coach participants through individual recovery plan development and implementation, assisting to build skills for self-direction and take control of their own lives.
  • Provide and facilitate referral pathways as the main goal.
  • Support implementation of services that best suit the participant's needs and wants.
  • Engage with NDIA regional contacts, including Local Area Coordinators.
  • Maintain positive working relationships and linkages with community agencies, clinical services, and other providers to ensure effective communication, referrals and support coordination can occur.
  • Monitor and report progress against NDIS Goal Plans, budgets, and data collection.
  • Maintaining direct interactions with NSW Public Health, Social workers, Support coordinators.
  • Meet targets for effective service provision and adhere to KPIs.
  • Provide and maintain quality service and portray a professional image.
