Job Title: Support Coordination Specialist

We are seeking an experienced Support Coordination Specialist to join our team. As a Support Coordination Specialist, you will work closely with individuals and their support teams to develop and implement personalized plans that promote independence and community engagement.

Your key responsibilities will include:

  • Conducting thorough assessments of individual needs and goals;
  • Developing and implementing tailored support plans that address these needs;
  • Maintaining regular communication with stakeholders and providing updates on progress;

To be successful in this role, you will require excellent interpersonal and communication skills, as well as the ability to work collaboratively with diverse groups. You should also have experience working in the disability sector and a strong understanding of the NDIS.
