Nurse Practitioner Aged Care Role

This is a rewarding position for an experienced Registered Nurse to contribute to a well-established aged care provider in Mackay. The successful candidate will be responsible for leading healthcare teams, delivering person-centred care, and contributing to continuous improvement.
Key Responsibilities:- Conduct baseline and focused assessments using a Positive Wellbeing Model.
- Develop and evaluate evidence-based care plans in consultation with residents and care teams.
- Monitor health outcomes and respond promptly to changes in condition.
- Accurately document care in accordance with electronic systems and legislation.
- Liaise with internal and external healthcare professionals to coordinate care.
- Provide leadership to care teams, fostering a culture of respect, collaboration, and clinical excellence.
- Support workforce management including mentoring staff and students.
- Ensure policy and compliance requirements are met, including reporting and quality documentation.
- Promote a zero-harm safety culture and drive continuous improvement across services.
- Actively contribute to business planning, risk management, and service delivery innovation.
- You are a Registered Nurse with a passion for aged care and a genuine commitment to improving the lives of others.
- You thrive in team environments, embrace leadership responsibilities, and are motivated by continuous learning and innovation in care.
