Job Summary

About the Role
  • This is an opportunity to develop and implement tailored care plans for residents, addressing their physical, emotional, and social needs.
  • As part of our multidisciplinary team, you will provide holistic care to residents, working closely with other healthcare professionals to ensure seamless delivery of services.
  • With a focus on resident-centered care, you will mentor and support care and service staff, enhancing their performance and helping them to meet the needs of residents.
Key Responsibilities
  • Develop and implement tailored care plans for residents, taking into account their individual needs and circumstances.
  • Work collaboratively with healthcare professionals to provide holistic care to residents, including assessing their needs and developing and implementing care plans.
  • Provide guidance and support to care and service staff, helping them to develop their skills and knowledge.
  • Identify and address training needs for clinical and care staff, ensuring they have the skills and knowledge required to deliver high-quality care.
Required Skills and Qualifications
  • Registered Nurse registration with the Nursing and Midwifery Board of Australia (AHPRA).
  • Experience in delivering clinical care within aged care or healthcare settings.
  • Strong planning, coordination, and mentoring skills.
  • Ability to guide staff in wound management, diabetes care, and medication systems.
  • Excellent communication, problem-solving, and teamwork skills.
