Job SummaryWe are seeking a highly skilled and compassionate Social Health Care Coordinator to join our team. As a key member of our team, you will play a vital role in shaping and achieving our organisation's vision and goals.
Key Responsibilities- Assess and triage clients who present or are referred to Social Health, determining the support required.
- Develop and implement comprehensive case management plans, delivering targeted supports to clients.
- Respond to critical incidents, providing wellbeing first aid to clients in distress.
- Liaise with community organisations and government agencies for information gathering, referral, networking, and promoting ATSICHS Brisbane services.

- Annual salary $74,351 to $82,964 (dependent on experience and qualifications).
- 11.5% Super + Not for Profit Salary Packaging (increases your take-home pay).
- Bachelor qualification in Social Work, Human Services, Behavioural Science, Social Science, Psychology, Criminology, Allied Health, AOD, Occupational Therapy, Sociology, or Public Health.
- Demonstrated ability to apply high-level practice judgement when working with clients.
- Ability to work with culturally and socially diverse groups of clients, applying a culturally responsive framework to guide interventions.
- Ability to work with diverse people and presentations, providing case-managed social work-driven supports.
- Willingness to travel across the ATSICHS Brisbane region and work across different sites.
- Working with Children's Card (Blue Card) or eligibility to apply.
- Disability Worker Screening Card (Yellow Card) or eligibility to apply.
- Current National Police Check.
- Previous experience in a multi-disciplinary team environment.
- Previous experience working in an Aboriginal Medical Service.
