Job Description

This role is an opportunity to make a meaningful impact in the lives of Aboriginal individuals and families by providing primary health care services. The Aboriginal Health Practitioner will work in partnership with clients to address their needs and provide support to promote healthy outcomes.

About the Role

The successful candidate will be responsible for delivering high-quality health care services to clients, including conducting health assessments, developing care plans, and coordinating with other health professionals to ensure comprehensive care.

Key Responsibilities
  • Conduct health assessments and develop care plans that meet the unique needs of Aboriginal individuals and families.
  • Provide education and support to clients on disease prevention, independent living skills, and respite availability.
  • Liaise with other health professionals to ensure comprehensive care and coordinate follow-up appointments as needed.
  • Maintain accurate and up-to-date records of client interactions, including medical history, treatment plans, and outcome measures.
Required Skills and Qualifications

The successful candidate will possess:

  • A Certificate IV or Diploma in Aboriginal Health.
  • Current registration with the Australian Health Practitioner Regulation Agency as an Aboriginal Health Practitioner.
  • Knowledge of current health issues impacting Aboriginal communities.
  • Well-developed written and verbal communication skills.
  • Experience working in a multi-disciplinary team environment.
