We are seeking a dedicated and compassionate Wellbeing and Health In-reach Nurse to join our team in Lismore. This exciting role is responsible for facilitating the identification of health, wellbeing and social needs, and coordinating appropriate early intervention, assessments, and referrals to services for school-aged children and young people attending designated primary and secondary schools.
About the RoleThe successful candidate will work in identified local schools, applying their specialist clinical nursing knowledge, experience and skill in providing complex nursing care for school-aged children and young people in these settings. They will be responsible for developing and maintaining relationships with local schools, healthcare providers and community agencies to ensure seamless service delivery.
Key ResponsibilitiesThis role involves:
- Conducting comprehensive assessments of students' health and wellbeing needs;
- Developing and implementing individualised care plans to address identified needs;
- Providing education and support to students, parents and school staff on health and wellbeing issues;
- Maintaining accurate records and reporting requirements;
- Participating in multidisciplinary teams to provide holistic care;
- Collaborating with other healthcare professionals to ensure consistent and high-quality care;
- Engaging with local communities to promote health and wellbeing initiatives;
- Monitoring and evaluating the effectiveness of services and making recommendations for improvement;

To be successful in this role, you will need:
