About the Role

As a Grade 2 Social Worker, you will have the opportunity to progress your career in a supportive environment and work with patients admitted to the General Medicine Units at a leading hospital.

This permanent, full-time position offers a dynamic and challenging work environment where you can utilize your skills and experience to make a positive impact.

Key aspects of this role include undertaking comprehensive psychosocial assessments, liaising with patients and their families, participating in quality improvement and research activities, and promoting positive relationships within the department and wider organisation.

  • Undertake multi-dimensional, comprehensive psychosocial assessments and apply clinical reasoning to formulate appropriate complex intervention and discharge plans.
  • Liaise with patients, their families and/or carers to co-ordinate, facilitate and communicate patient treatment and outcomes.
  • Actively participate in quality improvement and research activities to drive excellence in patient care.
  • Provide supervision and participate in clinical education programs as required.
  • Promote and develop positive relationships within the department, wider organisation and with community partners.
Your Qualifications and Experience
  • Bachelor Degree or Qualifying Masters in Social Work.
  • Significant social work experience in an Australian hospital setting is essential for success in this role.
  • Current National criminal history check, or willingness to obtain, is required.
  • Eligibility for membership of the Australian Association of Social Workers is also necessary.
