Job Title: Social Work Specialist

We are seeking a skilled and experienced social work specialist to join our team.

About the Role

This is an opportunity for a qualified social worker to provide advanced social work services to patients and their families, working in a multi-disciplinary team environment.

The successful candidate will have experience in conducting comprehensive psychosocial assessments, formulating and facilitating timely quality discharge plans, and providing casework expertise to patients.

Key Responsibilities
  • Liaise with health professionals, family members, and community agencies to coordinate care and support
  • Conduct thorough psychosocial assessments to inform care planning and ensure patient-centered outcomes
  • Develop and implement individualized care plans that address the physical, emotional, and social needs of patients
  • Collaborate with multidisciplinary teams to ensure seamless delivery of care and services
Requirements
  • Eligibility for membership of the Australian Association of Social Workers
  • Bachelor of Social Work or recognized equivalent qualification
  • Experience working in an acute hospital setting preferred
  • Excellent communication and interpersonal skills
  • Able to work effectively in a fast-paced environment
