Clinical Aged Care Specialist

We are seeking an experienced Clinical Aged Care Specialist to provide comprehensive assessments and support plans for clients. The ideal candidate will have a strong background in healthcare and aged care, with experience working directly with clients and their families.

The successful candidate will be responsible for undertaking thorough assessments, reviews and development of personalized support plans to assist clients in achieving their goals and living independently. They will also participate in clinical triage and case conferencing, identifying and managing risks as needed.

  • Develop and implement tailored support plans to meet the unique needs of each client.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
