Job Title: Care Coordinator
About the Role:
We are seeking a skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will work closely with individuals who require support to navigate services and meet their unique needs.
Key Responsibilities:
- Fulfill onboarding requirements for new staff members
- Assist participants in understanding and implementing their individual plans
- Coordinate services and supports in line with participants' goals and needs
- Act as the primary point of contact for participants, families, and service providers
- Build and maintain positive relationships with service providers, local communities, and other stakeholders
- Monitor and review participants' progress towards their goals and adjust support plans as needed
- Assist participants with plan reviews, plan changes, and accessing online resources
- Provide information, advocacy, and support to participants to ensure they are fully informed and empowered
Required Skills and Qualifications:
- Experience working with vulnerable populations or in the disability sector is highly desirable
- Strong understanding of relevant frameworks and Practice Standards
- Excellent communication and interpersonal skills
- Ability to manage multiple tasks and deadlines with attention to detail
- Compassionate, empathetic, and client-focused approach
- Ability to work both independently and collaboratively within a team
- Relevant qualifications such as community services, social work or related field
- Current Working with Children Check (or willingness to obtain)
- Current Police Check (or willingness to obtain)
