Job Title: Care Coordinator

About the Role:

We are seeking a skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will work closely with individuals who require support to navigate services and meet their unique needs.

Key Responsibilities:

  • Fulfill onboarding requirements for new staff members
  • Assist participants in understanding and implementing their individual plans
  • Coordinate services and supports in line with participants' goals and needs
  • Act as the primary point of contact for participants, families, and service providers
  • Build and maintain positive relationships with service providers, local communities, and other stakeholders
  • Monitor and review participants' progress towards their goals and adjust support plans as needed
  • Assist participants with plan reviews, plan changes, and accessing online resources
  • Provide information, advocacy, and support to participants to ensure they are fully informed and empowered

Required Skills and Qualifications:

  • Experience working with vulnerable populations or in the disability sector is highly desirable
  • Strong understanding of relevant frameworks and Practice Standards
  • Excellent communication and interpersonal skills
  • Ability to manage multiple tasks and deadlines with attention to detail
  • Compassionate, empathetic, and client-focused approach
  • Ability to work both independently and collaboratively within a team
  • Relevant qualifications such as community services, social work or related field
  • Current Working with Children Check (or willingness to obtain)
  • Current Police Check (or willingness to obtain)
