About the Role

The Community Support Liaison is responsible for providing clients with consistent, holistic support and increasing client satisfaction through regular interactions. The ideal candidate will offer complete support, understand the evolving clients' needs, and co-design services to meet client requirements.

  • Key Responsibilities:
  • Act as the primary point of contact and direct client requests to the appropriate department.
  • Undertake client re-assessments and regular reviews, developing, implementing, and evaluating support plans to ensure the service is responsive to the needs of clients to optimise their independence and wellbeing.
  • Maintain regular, empathetic communication with clients and their families to build trust and understand their concerns and expectations.
  • Address and resolve issues and concerns about care quality, staff interactions, facility amenities or other matters of care and service.
  • Collaborate with specialists internally and externally to ensure client care plans are up-to-date, comprehensive, and effectively implemented.
  • Gather and act upon feedback from clients and their families to enhance care services and facilities.
  • Act as an advocate for clients, ensuring their rights and interests are represented and respected.
  • Provide support and information to families and carers about aged care processes, options, and decision-making.
Requirements

We are seeking an individual with the following skills and qualifications:

  • Tertiary qualifications in Health, Social Work, Community or similar (Desirable)
  • Demonstrated experience in Aged Care, Disability or Community Care sector
  • Demonstrated ability to create and maintain client relationships in similar roles.
  • Ability to fluently speak a second language such as Cantonese and other languages (Desirable)
  • Demonstrated knowledge of quality standards within Aged Care, NDIS or Allied Health services.
  • Ability to communicate clearly and effectively.
  • Ability to plan, implement and evaluate services that meet the needs and interests of individual clients.
  • Ability to positively interact and collaborate with all stakeholders.
  • Critical thinking skills, including the ability to analyse and challenge conventional practice and ideas.
