
Young People's Support Specialist

We are seeking a dedicated and compassionate professional to provide case management services to young people at risk of or experiencing homelessness.
As a Case Manager, you will work within a restorative practice framework and cultural accountability to provide tailored, person-centred support to help clients achieve stability and independence.
Key Responsibilities:- Provide trauma-informed case management within the Tapa Marnini-Apinthi Practice Framework
- Coordinate internal and external referrals for housing, mental health, and other support services
- Advocate for young people and provide personal, emotional, and cultural support
- Maintain accurate client records and contribute to data reporting and continuous improvement
Requirements:
You will require a tertiary qualification in Human Services (e.g. Social Work, Youth Work, Psychology, Community Development) or Diploma in the related field of study. Alternatively, extensive relevant experience may be considered in lieu of qualifications.
Experience working with young people, Aboriginal people, and communities in the provision of support services is essential. You will also need to have experience working with individuals who have experienced grief, mental health issues, dislocation, poverty, homelessness, marginalisation, poor health, and undiagnosed or untreated medical conditions or disabilities.
