
Family Support Professional

You are applying for the position of Home Based Care Practitioner. The role involves monitoring and supporting the placements of children in Kinship and Foster care situations, working closely with carers and providing Case Management.
This is a part-time opportunity located at our Ballarat office. You will be part of our dynamic Home Based Care Team, working autonomously and collaboratively to make a positive impact on vulnerable children's lives.
Required Skills and Qualifications- A tertiary qualification in Social Work, Welfare, Community Services or a related field
- Experience working with vulnerable young people and case management
- An understanding of Foster and Kinship care arrangements
- The ability to build good working relationships and rapport with clients, colleagues and other stakeholders
