Job Title: Extra Care Scheme Manager

Overview:

The role of the Extra Care Scheme Manager is to oversee the provision of on-site care and support services within the scheme, ensuring residents receive a high standard of service. This involves promoting their independence, security, and wellbeing through effective partnership working.

Main Responsibilities:

  • Developing and implementing person-centered care plans in collaboration with relevant stakeholders.
  • Maintaining relationships with social workers, commissioners, health authorities, and other professionals involved in the integration of the service.
  • Leading investigations into complaints, safeguarding concerns, or incidents and ensuring responses adhere to service level agreements.
  • Supporting staff development through training, performance reviews, and mentoring.
  • Collaborating with housing providers to deliver activities and promote resident participation.
  • Liaising with healthcare services for administering, recording, and supervising medication according to established policies and procedures.
  • Providing direct care and support to residents in emergencies as needed.
  • Engaging with various stakeholders, including care workers, families, and advocates, to ensure comprehensive resident care.

Key Requirements:

  • Understanding the needs of older adults and the purpose of Extra Care Housing.
  • Knowledge of best practices and innovations in Extra Care settings.
  • Successful leadership experience in care and support services.
  • Able to produce accurate reports and meet deadlines in line with contractual and regulatory obligations.
